Laporan

Penerima izin/penyelenggara undian berkewajiban menyampaikan laporan secara tertulis mengenai hasil penyelenggaraan undian dalam waktu paling lambat 30 (tiga puluh) hari setelah penyerahan/pemberian hadiah kepada para pemenang.

Laporan disampaikan kepada Menteri Sosial RI dengan tembusan Gubernur dimana pengundian dilaksanakan dengan melampirkan:
Berita Acara Pelaksanaan Pengundian untuk Undian Gratis Berhadiah Tidak Langsung dan/atau Berita Acara Penyegelan untuk Undian Gratis Berhadiah Langsung yang disahkan oleh Notaris.
Daftar pemenang hadiah undian dan bukti tanda terima/penyerahan hadiah serta fotokopi KTP/identitas diri lainnya yang masih berlaku dari pemenang.
Daftar hadiah undian yang tidak diambil oleh pemenang/hadiah tidak tertebak atau tidak ada pemenangnya (apabila ada).
Tanda bukti penyetoran pajak penghasilan atas hadiah undian ke Kas Negara setempat melalui bank persepsi dengan menggunakan Surat Setoran Pajak Final (SSP Final).
